How to report a near miss
PURPOSE
A near miss is an incident that could have caused injury or damage but did not.
WHY REPORT IT
Near misses reveal hazards before someone gets hurt.
WHAT TO INCLUDE
Describe what happened, where it happened, what could have gone wrong, contributing conditions, and any photos if safe.
EXAMPLES
Trips without injury, falling objects, blocked exits, electrical hazards, or almost-collisions.

Near misses are free warnings. Use them.
