How to prepare an emergency contact card

PURPOSE An emergency contact card gives responders or workplace contacts important information during urgent situations. WHAT TO INCLUDE Name, emergency contact name, relationship, phone number, important medical notes if you choose to include them, and preferred contact method. KEEP IT CURRENT Update the card after phone, address, relationship, or medical changes. PRIVACY Only include information you are comfortable sharing in an emergency. BEST PRACTICE Keep a copy somewhere accessible but not broadly exposed.
